Stage 1

Initial Referral
Initial Referral and Placement Prospect Assessment Conducted by the Responsible Individual and Registered Manager
• Upon receiving the referral documentation—along with any additional information provided by supporting agencies—the matching process is initiated. A comprehensive Impact Risk Assessment is completed to determine whether the child or young person's needs align with the service’s capacity, and to assess any potential impact on the existing residents.
• Discussions are held with both the current young people living in the home and the staff team to review the potential admission. The outcome of these discussions will determine whether the referral proceeds to Stage 2.

Stage 2

Informal Visits
Introduction and Fact-Finding by the Registered Manager and Team Member
• If deemed appropriate, the Registered Manager, accompanied by a team member, will arrange a visit to meet the young person in their current home. This initial introduction offers an opportunity to explain our service and outline what our provision entails.
• We will actively support and encourage the young person—along with their parent, carer, or Social Worker—to attend an informal visit to our home. This visit allows them to explore the environment, meet members of the staff team, and interact with some of the current residents.
• If the visit results in a shared positive outlook—where both the young person and our staff feel the placement would be in the best interest of the child—the referral will proceed to Stage 3.

Stage 3

Formal Visits
Key Worker Assignment and Pre-Admission Planning Meeting
• Where appropriate, additional visits to the home will be offered to further support the young person’s transition.
• To ensure the child or young person is actively involved in shaping their care, a Key Worker will be assigned through consultation with the young person and their Social Worker.
• A pre-admission care and placement planning meeting will be arranged to finalise preparations for the placement.

NW Care Group has six children’s homes located across the Trafford area, providing safe, supportive, and nurturing environments for young people. Each home is carefully situated within local communities, ensuring that children have access to schools, recreational facilities, and other essential services, while also benefiting from a stable and caring setting.
